2. Project Description (maximum 3 pages)

Include:

• Background and significance

• Specific aims

• Research approach

• Expected outcomes

• Future funding plans

3. Budget and Justification (maximum 1 page)

Provide a brief budget describing how award funds will be used.

4. Institutional Support Letter (maximum 1 page)

Letter from department chair, division chief, institute director, or equivalent confirming:

• Institutional support

• Protected time (if applicable)

• Availability of required resources

5. Mentor Letter (maximum 1 page, if applicable)

For applicants with identified mentors, describe:

• Mentoring plan

• Scientific oversight

• Career development support
